Implement md5(bytea), update regression tests and documentation. Patch 

from Abhijit Menon-Sen, minor editorialization from Neil Conway. Also,
improve md5(text) to allocate a constant-sized buffer on the stack
rather than via palloc.

Catalog version bumped.
