Make worker_spi sample code more complete

Make use of some GUC variables, and add SIGHUP handling to reload
the config file.  Patch submitted by Guillaume Lelarge.

Also, report to pg_stat_activity.  Per report from Marc Cousin, add
setting of statement start time.
