Corrections to patch adding SQL/MED error codes.

My previous commit, 85cff3ce on
2010-12-25, failed to update errcodes.sgml or plerrcodes.h.  This patch
corrects that oversight, per a gripe from Tom Lane, and also corrects
a typographical error.
